Breakthrough bleeds whilst on the pill?

For the past 3 or 4 months i have been bleeding on my 3rd week of taking the contraceptive pill? What could be causing this. Somethimes its just as if my period comes a week early (instead of my pill free week) but this month its only a little blood! What does this mean
